In general, the basic requirements that make a person with Multiple Sclerosis or Parkinson’s Disease eligible for a powerchair  or scooter under Medicare/Medicaid include: 
 

  • A correct diagnosis. A diagnosis of MS or Parkinson’s Disease must indicate that the patient is incapable of operating a manual wheelchair as a result of a severe neurological or muscular condition of both arms and legs.
  • Proof of functional impairment – severe weakness of the arms and legs that make it impossible for the patient to use a manual wheelchair.
  • Certificate of medical necessity (CMS). This is one of the most vital pieces of paper when it comes to determining your eligibility for a powerchair or scooter.
  • A written, detailed patient evaluation that documents the progression and prognosis of the disease and other information related to the patient.
